Before we begin our care, we discover who you or your loved one is as a person, not just a patient. We want to learn about what makes you, you. Your personality, hobbies, what foods you love are just some examples. With knowing you as a person, and the needs you have, your customized “Branches of Care” Plan is then designed for you, by our Registered Nurse.
